Yet another reason to wish Sarah Palin would just go away forever.

The Congressional Budget Office projects that the average annual growth in Medicare spending will be 5.8 percent between 2012 and 2020. It would have been one percentage point higher than that, according to the CBO, if not for the cost-constraining provisions of the Affordable Care Act, most notably the one that will gradually eliminate the bonuses the government pays private insurers to participate in the Medicare Advantage program. 

The Affordable Care Act might have been able to curtail spending further if it hadn’t been for Sarah Palin’s reckless rhetoric. It was Palin who charged that a provision of the law allowing Medicare to pay doctors for having end-of-life discussions with their patients would lead to government-run “death panels.” 

That provision was important because, according to the Congressional Research Service, about one-fourth of total Medicare spending is for the last year of life, and a lot of that spending could be avoided if more folks received counseling from their doctors on what they should do to ensure that their wishes are carried out when the grim reaper comes calling. 

The principal sponsor of the legislation, Rep. Earl Blumenauer, D-Ore., said he was stunned when the controversy erupted. “It’s just beyond bizarre,” he told reporters at the time, noting that his bill had broad bipartisan support before Palin posted the death-panel charge on her Facebook page. 

What was a good idea then is a good idea now, but Palin so poisoned the well that not a single Republican, not even Boustany, will go near it, certainly not in an election year. Blumenauer has reintroduced the measure as a stand-alone bill, and it has several cosponsors. But as you might imagine, all of them are Democrats. And because Republicans now control the House, Blumenauer hasn’t even been able to get a hearing on the measure. 

Today Palin's influence is far less impactful then it was back in 2009, but the damage her lunatic ranting have caused still lingers on.

And the truly sad thing is that in many ways her influence can still be felt in the kind of derisive, entrenched partisanship that rules the day in our current political atmosphere.

    Anyone who's had a family member fall sick, without the hope of recovery,almost always wants to hear from doctors what their options are.

    The point is not to cut off care that is needed, but not to prolong a life that nearing its end, usually painfully. So many people have not written "end of life" directives (figuring, I guess, that they'd never die), often his or her family is at sea about the best thing to do to care for the patient.

    That's what this is all about. No "death panel" is going to terminate a life. The meeting is just to make sure that everyone understands all the possibilities, risks, and pain involved at the end of life.

    But, yeah, with one stupid quip, of which I'm sure she's extraordinarily proud of herself, Palin made these very necessary and personal decisions into a political slogan, to the detriment of everyone.

    Perhaps when she's at the point that she needs help at the end of her life, her "family," will be too busy to bother with her, or too eager to unplug her so they can inherit her money. A "death panel" would be a necessity for her, then.

  11. My elderly neighbor was diagnosed with lung cancer in late fall and told he would probably not see Christmas. He did some chemo to manage pain but decided not to go the route of aggressive radiation and just let nature take his course. He had a few incidents requiring blood transfusions and called an ambulance about a week and half before he died in April to get to the hospital for a transfusion, but other than that he was focused on getting to the Pearly Gates with as little pain as possible. He died peacefully and at peace. Why one would want to put a huge amount of radiation into a man in his final few months of life to extend his suffering a few more months if the patient says otherwise is beyond me. But as long as there is media attention to be had and grifting to be done, Grifter's Gonna Grift. Stupid heartless ignorant tuat.

She didn't have an alternative plan to a provision for end-of-life discussions that are very important to elderly patients. Instead, she cursed the plan, with no willingness to suggest other ways, or cared one iota about the fear she generated in the elderly. A lot of these people were being made to believe this was some mandatory plan to invade their end-of-life wishes. The woman curses with her mouth way too much and it's time someone calls her out on it. She got away with those "death panels". She is mean, nasty, and must be a very very very unhappy individual.

    Sarah Palin has not been a politician ever since she quit her job as governor. She is as ignorant as the day that McCain picked her as his VP running mate. There is no reason why her opinion should matter any more than the opinion of anyone else who is a celebrity. I don't care who Kim Kardashian is going to vote for. I don't care who Paris Hilton is going to vote for and their endorsements and comments don't mean a thing. Sarah is in that same group of people, a celebrity, doing and saying outrageous things just to get noticed.

    Sarah was the one who wanted to push that Reverend Wright stuff, when McCain didn't think that it was a good idea. Sarah only knows how to say nasty things. You can tell that it was a bad idea when even Romney had to back away from trying to use Reverend Wright again. Didn't work then, and won't work now. That's Sarah's game: name calling. And she is the one who stuck the knife in the heart of health care reform. Thanks to her celebrity, she can afford good medical care for her family-- however a few years ago, because Sarah was the star of reality TV, she would not have been able to pay for catastrophic care for her own parents and/or support Trig's care and therapy. No wonder she cashed in.

    Will someone please tell me how she has managed to immobilize 435 sitting U.S. Representatives in this matter? Who (or what) shapes her sphere of influence in such a way that no one will meet her head-on and flush her out? Is it the Koch Brothers? Even in the movie "Game Change" John McCain, a sitting U.S. Senator, for Gods sake, was afraid of her! At the end of the movie he said he was afraid she would turn on him too!
    Why?
    She has been shown time and time again to be a buffoon, an intellectual desert, among many other things, and yet people are afraid of her?

    Just going away quietly isn't enough. We...at least, I...need closure.

    The Alaska legislature could help the public by investigating the conflicting Troopergate findings. Or the Personnel Board could reopen their investigation with the revelations from Frank Bailey's emails--the ones they were supposedly missing during their investigation. A perjury violation is being ignored.

    The media could insist on an investigation into the APD corruption and accusations against Todd Palin.

    The media could insist that Palin answer the babygate questions head-on with objective documentation of her claimed pregnancy and Trig's birth.

    Is there anything we can do to interest either the AK legislature or the media into doing their jobs on behalf of the public?

    Is there any point in pursuing the loose ends in the existing babygate evidence? I think it's unlikely we could get definitive proof of a hoax without cooperation from someone directly involved but is there an angle to the story that would demand media attention to pressure Palin for answers or at least provide widespread coverage of the questions?

Publically question how MatSu allowed a high risk birth at a hospital not equipped for high risk births . We need constant relentless pressure for those responsible to Answer these questions .       Couldn't someone sue the hospital where Trig was supposedly delivered, bue to the extreme risk taken? This was a high risk pregnancy, with the premature birth of a down syndrome child with a hole in his heart, to a mother of advanced age and a history of miscarriages? My twins couldn't be delivered at the hospital my doctor usually delivered at because it wasn't a trauma level 4 hospital that was equipped for a high risk birth. Given that the Mat-Su hospital where she claims the birth took place was not a Trauma Level 4 hospital, no doctor's malpractice insurance company would DARE allow the doctor to deliver there. The hospital would be forced, in court, to admit he wasn't born there to protect themselves.

      What "conflicting" findings?
      You've read too many blog comments putting bullshit out there, incorrect statements and false information, instead of phrasing their stuff as their opinion or a question.

      The "Troopergate findings" were that Sarah was in violation. THAT was the finding. She was found to have acted improperly. Even without Todd and her staff and key persons that were physically present at meetings. That was the whole point of all these people refusing subpoenas. It was worth it to be faced with contempt charges. And they all were. Except, no consequences. It worked.

      The REAL question is why she was allowed to negotiate her way out of being held accountable.
      That was entirely a perk of being selected as McCain's VP. Can't take a campaigning VP to task, can ya? Then what, if McCain won?
      Then the shit would really hit the fan, wouldn't it?
      No, folks, the answer is that Alaska is dominated by the Republican Party.

    Argument over at at "The P" regarding teen virginity. One poster says it's not important, two very prominent commenters, that support Palin and Bristol wholeheartedly, are speaking of the importance of remaining a virgin until marriage and are having some sort of competition over whose daughters are more virgin. The hypocrisy over there is absolutely astounding! I haven't been over there for a while but the mind certainly boggles over what is said...someone asked one commenter to please interview Sarah when she is in Texas for some sort of political shindig. He basically replied that he wouldn't want to interview her and put her "on the spot" as "she works in mysterious ways" and much of what she does is kept under the carpet until such a time as she sees fit to engage! They're all opening their wallets for this latest moneygrab because she "has their backs"! I just love it, if I'm feeling sad our blue, visiting the "P" certainly makes the day a little brighter ;-)

She's a little disappointed and confused that none of the the recent money is going towards updating the SarahPac website so that the loyal followers can keep abreast of which new batch of candidates their donations are "going to" (answer, none of them....)

    I am still disappointed in the SarahPAC website. It is still displaying her winning picks from 2010 and not a single endorsement for 2012. I have written the SarahPAC website 5 times now asking when they are going to update the site and have not once received a reply back. So there is no Deb Fischer or Mourdock on the SarahPAC site as current picks. This does NOT reflect well on Sarah. It has the appearance of her not participating in the 2012 elections. There are no write-ups by Sarah about the objectives for 2012. Nothing. The website is where part of our money is going and it hasn't changed in months.

    I would encourage everyone to write an email to request that the site be updated with Sarah's 2012 picks. Select "General Contact" from the drop down menu.

    My 99-year-old mother got a dx of pancreatic cancer 6 weeks ago. She visited her primary care physician the day after scans in the ER showed the tumor and metastases. He told her her options...not very well or thoroughly, but enough for her to think about and discuss with my sibs and me. He told her to make an appointment for 2 weeks, which she did, and where she told him she would be doing nothing about the cancer and where she got reassurances that she could be kept free of pain (uh, not quite!). As far as I know those visits will be covered by Medicare. Or her supplemental plan. I may be wrong. But then the day before Mother's Day my mom ended up in the ER with really really severe abdominal pain, and her primary care physician stayed out of the picture even though we called him. He said the ER personnel were very good at handling pain and that my mom was in good hands. Was that because he would not be reimbursed by taking charge at that point? Again I don't know. But we thought it was odd that he seemed to be washing his hands of her case. She's under hospice care now with him as the primary doctor, but really all he does is OK medication orders.

      I'm so sorry, but at 99 and with pancreatic cancer there is NO good outcome. If she is on hospice care (and the hospice is reasonably good)and making sure she has pain medications- which is something those hospice nurses are really good and experienced at- then that is all that can be done.

      I think what the bill was talking about what what I overheard one day on my job. That day I was working doing admissions and recoveries for the GI lab- people having endoscopies, etc. One man came in for a look at his pancreas. After the procedure, the doctor came in, pulled the curtain around the bed, and sat down with him. He explained carefully that what he had seen was the cancer growing, and that there was no form of treatment that would do anything but prolong the dying process. If the patient had something he really wanted to stay alive for, such as the birth of a grandchild or something, they would work for that goal. But he really wanted to make sure the patient understood that this information did not change the patient's condition in any way. The cancer was there, all they did was confirm the extent of it. The patient said he just wanted to be kept as comfortable as possible, and his wife wanted him to make it past Christmas and into January, because she didn't want their kids to always be sad at Christmas. The doctor said that he thought the patient had a few months left, and he would do anything possible to keep the patient as comfortable as he could. He gave them his office number: the back line, not the front desk. He said that they could call him at any time and he would do anything he could to support the patient and his wife during this time. They all hugged and the doctor left.

      Now, I know, if there had been anything that could reasonably have been done to effect a cure, the talk would have gone differently. The patient would have been referred to an oncologist. As it was, the doctor spent time with the patient, and when he left, the patient not only understood the options, but what was going to happen and how he could get through it, and most importantly, he understood that the doctor cared about his wishes and about keeping him as comfortable as possible.

      That probably got billed as "discussion of findings" or something. But it was end of life care counseling, and there are assholes that don't think the doctor should have been paid for it. I think the doctor spent 30 minutes providing true care.

      Here's the thing: the righties are totally against hospice care. In fact, they call hospice care "euthanasia". They scream that it's "anti-life", and have lobbied angrily against it in various states. Remember the Terri Schiavo case??

      So, the righties do not want you to have health care that would PROLONG your life (help heal you while you're young) and, when the time comes for you to die, they also do not want you to have health care that would let you END your life painlessly, easily, with dignity and privacy, and without bankrupting your family.

      How ironic: the righties are the ones who shriek about the nanny state, but they are the ones who insist on having the right to control all of our personal decisions.

      Our doctor's practice was sold to an investment group, just like all of the other companies that have been bought and sold. Our doctor no longer practices at the hospital. If we require hospitalization, we can go to any of the hospitals in the group, and he will hand us over to the appropriate hospital doctor. I don't like this one bit, but it must distance the doctors in the new practice from having the carry expensive insurance in the case of surgery, for example. Everything today is being done for the bottom line, not for the care of the patient, I am sorry to say.

      I worked in a hospital that had started to bring in "hospitalists." They are MDs with specific training toward treating patients needing hospitalization. This means that the patient's regular physician or pediatrician may not see them in the hospital, they would instead be seen by the hospitalist. Once they are released from the hospital, they would be under the care of their physician, who would get all the reports on what was undergone during hospitalization.
      The rationalization is that these MDs are better equipped and trained to oversee patients with more extensive conditions. They are more experienced in, say, the patient with severe diabetes requiring hospitalization than the patient who is managing their illness. It is meant to provide better care, and because hospitalists are more versed in sicker patients, they are supposed to provide better care that shortens length of stay and improves outcomes. It also is a means to providing a better quality of life to physicians. (Did you know there are many specialties that are facing a severe shortage of doctors?) Having a hospitalist means your primary care doctor can continue to see you but your care is provided by a specialist in treating hospitalized patients rather than a GP having to run to the hospital when his or her patient is sick enough to require hospitalization. I can't say this is always the case, but thought you might be interested in the rationale.

      When you are admitted to the hospital, especially the ER, you are in the province of the hospitalizes and the ER people. I spent several hours in our hospital's ER while my symptoms were being analyzed from all directions. I was told by a nurse that my internist had stopped by but I never saw him. I did see him two weeks later. He had all of the test results but no one was sure what had caused my extreme symptoms. Thankfully they've not recurred. I always thought it had something to do with the infection I was being treated for - another phase before I finally got better. Who knows but the family doctor does not treat the patient in the ER.

    One of my parents has a health care directive, the other does not.

    The parent that does has given precise instructions on just how far we are to go in prolonging life, and there is some comfort in knowing that we will be abiding by those wishes, instead of our own, if it comes to that.

    Should the other parent, who is no longer considered capable of making those decisions, be in dire medical straits, the medical staff must do all they can, even if they believe that the quality of life will be unacceptable in terms of cognitive disability and physical pain for the patient. They have already made it clear they do not wish to do so, but currently have no alternative. This is not what my parent would have wanted. Not only would we treat the family dog better, but the taxpayers will get to pick up the tab for this inhumane treatment.

    Her continued failure to retract her statements and set the record straight on this issue is unconscionable, but then, she's proven time and again that she has no ethics, no conscience, and no problem throwing away other people's money.

    The "death panels" are just payment option for medicare doctor appts. It is a way for doctors to receive a higher reimbursement rate for discussion with patients regarding living wills/advanced directives. It can be time consuming and many docs don't do it because of time constraints. So this "extended visit" gives the opportunity for the doctor to be clear on a patient's wishes PRIOR to a hospitalization when the patient may be unable to convey his/her wishes.

      It is also something that has been recommended by lawyers and doctors for the last twenty-five years or so. I used to work for a major midwest law firm and when living wills/advanced directives became an option, many years ago, our firm urged everyone to do so. My husband and I did right away. When we retired and moved to our present community, the local hospital asked for a copy as soon as either of us arrived for routine testing. In fact the hospital has its own form for doing medical directives and they are handed out to anyone who shows up at the door without a completed form. Why the ABA and the AMA and the AHA did not rise up and decry the "death panel" nonsense is just another mystery to me. (Actually, I suspect it was because these organizations have heavily Republican roots, but I try not to think about that.)
